Overview:

Every system has a goal. Some systems may have more than one. The purpose of each part of a system is to successfully execute its task or tasks in order to support achieving that system goal or goals.

Management of the design and operation of a system is to mitigate against failure of each part of a system in order to ensure the system goal is achieved.

The human operator is an integral part of a system. As with any other part of the system, the human operator can fail. Understanding why a human failure may occur is the first step to introducing effective mitigation.

From there we can target the root causes of human error and select the most appropriate means of mitigation in terms of effectiveness, cost and any intended impacts.

Why should you Attend: Estimates and statistics on the percentage of major incidents for which human error is a contributing factor vary. However, it is consistently over 50% and sometimes as high as 90%. Therefore, it is important to establish an understanding of how human error originates and the influencing factors around the prevalence of human errors.

Speaker Profile
Ben Woodcock is a Human Factors Consultant with over 20 years experience of applying his discipline across several industries, including Rail and Transportation, Energy and Defense. He has operated within several countries including the UK, USA, Canada and UAE. He is currently the Project Delivery Director at SNC Lavalin’s Rail and Transit Division, based in Calgary, Canada.

Ben has contributed Human Factors leadership to many major design projects and operational systems. Examples of high profile projects Ben has worked on in recent years include the Eglinton Crosstown Light Rail Transit, Olympus TLP, Big Foot TLP and the Channel Tunnel Rail Link. Over the course of his 20 years experience he has developed an understanding of an optimized approach to integrating Human Factors throughout design and operating processes.

Ben is the author of several published papers in the fields of Risk Management and Human Factors. He has contributed to several Process Safety textbooks. Ben earned his BSc. at Loughborough University in the UK and is a Chartered Member of the Institute of Ergonomics and Human Factors.
